Breast augmentation is breast enlargement, also called breast enhancement, usually done with insertion of saline or silicone implants. Usually performed at a surgicenter or hospital with general or twilight anesthesia and use of a recovery room. See photos of Patients 1 through 6, below, for before and after Breast Augmentation.
Mastopexy is a breast lift, which is essentially skin excision to tighten the skin envelope around the already present breast tissue. Can be performed as an office surgery under local anesthesia with sedation. The patient shown here chose Mastopexy as part of her procedure.
Positioning of Implants -- Sometimes implants are placed over the muscle; sometimes underneath. The choice depends on each individual patient's current condition, age, breast size and objectives. Augmentation-mastopexy is for the patient who wants both larger and lifted breasts. If the breasts are too saggy to lift by implants alone, then the patient needs an augmentation-mastopexy. This dual procedure can be performed in one or two stages.
In one stage, there is just one operation but the two parts of the operation tend to work against each other. The augmentation is stretching the tissues apart, while the mastopexy is pulling the skin inward, so the scars from the skin excision mastopexy tend to be wider and more visible when it is done in one stage.
An alternative is to perform the dual process in 2 stages, first lifting the breast alone (without the extra stretch on the skin and inner tissues because no implants are inserted in stage one), and then several months later, the augmentation can be performed, to increase the breast size. This patient chose augmentation-mastopexy in a two-stage procedure.
